To access services an individual may come to our campus at 1004 N. Pearl Street in Paola, Kansas.

Some services require the completion of some initial paperwork. These applications may be completed before coming to our office or they may be completed and faxed or sent in via the postal service.

Transitional Housing Application
If you are interested in entering our Transitional Housing Program, please use the link above to access the application and then print it.

If there are signatures required on the application, please send the original signed copy in if you choose to fax it. Faxing it will help us begin planning for your request but requests will not proceed without the original signed form.

Transitional Housing
Our facility provides housing to individual women 18 years and older, younger emancipated women and families who are experiencing homelessness. Applicants must be experiencing homelessness.

An application must be completed and eligibility determined before entering the transitional housing program. Eligibility is, in part, determined by the definition of homelessness adopted by the Department of Housing and Urban Development.
